ABSTRACT

Objective: To examine the clinical characteristics and prognostic factors of elderly patients with mantle cell lymphoma (MCL) and the impact of nutrition and underlying diseases on the prognosis of elderly patients with MCL. Methods: retrospectively analyzed 255 elderly patients with MCL from 11 medical centers, including Peking University Third Hospital between January 2000 and February 2021. We analyzed clinical data, such as age, gender, Mantle Cell Lymphoma International Prognostic Index score, and treatment options, and performed univariate and multivariate prognostic analysis. We performed a comprehensive geriatric assessment on elderly MCL patients with medical records that included retraceable underlying disease and albumin levels, and we investigated the impact of basic nutrition and underlying disorders on MCL prognosis in the elderly. Results: There were 255 senior individuals among the 795 MCL patients. Elderly MCL was more common in males (78.4%), with a median age of 69 yr (ages 65-88), and the majority (88.6%) were identified at a late stage. The 3-yr overall survival (OS) rate was 42.0%, with a 21.2% progression-free survival (PFS) rate. The overall response rate (ORR) was 77.3%, with a 33.3% total remission rate. Elderly patients were more likely than younger patients to have persistent underlying illnesses, such as hypertension. Multivariate analysis revealed that variables related with poor PFS included age of ≥80 (P=0.021), Ann Arbor stage Ⅲ-Ⅳ (P=0.003), high LDH level (P=0.003), involvement of bone marrow (P=0.014). Age of ≥80 (P=0.001) and a high LDH level (P=0.003) were risk factors for OS. The complete geriatric assessment revealed that renal deficiency was associated with poorer OS (P=0.047) . Conclusions: Elderly MCL patients had greater comorbidities. Age, LDH, renal function, bone marrow involvement, and Ann Arbor stage are all independent risk factors for MCL in the elderly.

@#Abstract: Objective To analyze the industry characteristics of new occupational pneumoconiosis in Foshan from 2007 to Methods 2021. The data of new cases of occupational pneumoconiosis in Foshan City from 2007 to 2021 were obtained from “ ” , “ the China Disease Prevention and Control Information System subsystem the Occupational Disease and Health Hazardous ”, Results Factors Monitoring Information System and the industry characteristics of the cases were analyzed. From 2007 to , , ( ) 2021 there were 886 new cases of occupational pneumoconiosis in Foshan mainly include silicosis 68.3% in males ( ), ( ) 94.2% and stage Ⅰ pneumoconiosis cases 75.9% . The median age of cases at the time of diagnosed disease was 47 years , old and the median working age of dust exposure was 11.2 years. The enterprises where all cases are located involve eight top , ; , - industry categories of which manufacturing industry accounted for 97.6% and 30 major industry categories of which non ( ) , metallic mineral products industry was the highest 60.5% . There were 554 sporadic cases and 332 cluster cases and the , , Conclusion industries were mainly non-metallic mineral products accounting for 55.0% and 69.6% respectively. , - Occupational pneumoconiosis in Foshan is dominated by silicosis and the key industries are non metallic mineral products.

ABSTRACT

Objective: To explore the clinical characterist ics and risk factors of hemorrhage complicated by hemoperfusion therapy in patients with acute poisoning. Methods: In January 2021, the clinical data of 196 patients with acute poisoning who received hemoperfusion therapy in the Second Affiliated Hospital of Air Force Military Medical University from January 2018 to December 2020 were analyzed, and the patients were divided into bleeding group and non-bleeding group according to whether the patients were complicated with bleeding. Multivariate logistic regression was used to analyze the independent risk factors for hemorrhage in patients treated with hemoperfusion. Results: A total of 21 patients in the bleeding group and 175 patients in the non-bleeding group were included. There was no significant difference in general data such as gender, age, and body mass index between the two groups (P>0.05) . Organophosphorus pesticides (χ(2)= 4.56, P=0.030) , HA230 perfusion device (χ(2)=4.12, P=0.042) , platelet count (t=-2.33, P=0.009) and activated partial thromboplastin time (t=14.53, P<0.001) at 2 h of perfusion were the influencing factors of hemorrhage in patients with acute poisoning treated with hemoperfusion. Among them, organophosphorus pesticides, 2 h perfusion activated partial thromboplastin time ≥35 s and other factors were independent risk factors forcomplicated bleeding (P<0.05) . Conclusion: Patients with acute poisoning, especially organophosphorus pesticide poisoning, are at greater risk of bleeding during hemoperfusion therapy. Monitoring of changes in activated partial thromboplastin time should be strengthened and the dose of anticoagulants should be adjusted in time to reduce the risk of bleeding.

ABSTRACT

OBJECTIVE@#To analyze the polymorphism of the HPA1-5,15 system of the donors in Zhangjiakou area.@*METHODS@#DNA was extracted from the blood samples of the donors, PCR- SSP method was used to divide HPA1-6, 15 genotype. The gene frequency and genotype frequency were calculated, compared with the difference and regiahal specificity of the populations in our country and foregiens was compared other populations.@*RESULTS@#The gene expression in the HPA-1, HPA-2 and HPA-4 systems were all homozygous aa, and the donors who expressed homozygous bb was not exessed. Among them, one heterozygous ab expression was found in both HPA-1 and HPA-4 systems (1%), and 14 cases of heterozygous ab expression were found in HPA-2 system (14%). The gene expression in the HPA-5 system was mainly homozygous aa (98%), and a very few expressed homozygous bb (2%) was found. The degree of heterozygosity of gene expression in the HPA-3 and HPA-15 systems was relatively high. The proprotion of the expression of aa, ab and bb in the HPA-3 system was respectively 46%, 40% and 14%, the proprotion of the expression of aa, ab and bb in the HPA-15 system was respectively 21%, 64% and 15%.@*CONCLUSION@#The gene frequency of platelet-specific antigen HPA1-5,15 system in zhangjiakou region shows local characteristics. The heterozygosity degree of gene expression in the HPA-3 and HPA-15 systems are both high, suggesting that they are more likely to result in alloimmunization and ineffective platelet transfusion, which should be pays attention to.

ABSTRACT

Objective:To observe the regulatory effect of modified Erzhu Erchentang on metabolization of polycystic ovary syndrome (PCOS) with spleen deficiency and phlegm dampness syndrome. Method:Patients 140 cases were divided into control group and observation group. Both groups were given metformin hydrochloride tablets, 500 mg/time, 3 times/day. Control group was given Yuejun Erchen pills, 0.5 g/time, 3 times/day, while observation group was given modified Erzhu Erchentang, 1 dose/day. The course of treatment lasted for 24 weeks. Before and after treatment, levels of fasting blood glucose (FBG), fasting insulin (FINS), glycosylated hemoglobin Alc (HbA1c), 2-hour postprandial blood glucose (2 h PG), blood lipid, waist circumference (WC), body mass index (BMI), waist hip ratio (WHR), luteinizing hormone (LH), follicle stimulating hormone (FSH), serum testosterone (T), estradiol (E<sub>2</sub>), dehydroepiandrosterone sulfate (DHEAS), sex hormone binding globulin (SHBG), leptin (LP), adiponectin (APN), resistin, visfatin and tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) were detected. Homeostasis model assessment insulin resistance (HOMA-IR) was calculated, modified Erzhu Erchentang was scored, and recovery of menstruation and ovulation and ovarian volume were recorded. Result:Levels of FBG, 2 h PG, HbA1c, FINS, HOMA-IR, triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), LH, FSH, T, E<sub>2</sub>, DHEAS, LP, resistin, visfatin and TNF-<italic>α</italic> in observation group were lower than those in control group (<italic>P</italic><0.01), and levels of BMI, WC and WHR were lower than those of control group (<italic>P</italic><0.05). And levels of high-density lipoprotein cholesterol (HDL-C), SHBG and APN were higher than those in control group (<italic>P</italic><0.01). Score of modified Erzhu Erchentang was lower than that in control group (<italic>P</italic><0.01), and ovarian volume was smaller than that in control group (<italic>P</italic><0.01). The normal rate of BMI was 49.23% (32/65), which was higher than 30.30% (20/66) in control group (<italic>χ</italic><sup>2</sup>=5.151, <italic>P</italic><0.05). The normal rate of blood lipid was 93.85% (61/65), which was higher than 81.82 % (54/66) in control group (<italic>χ</italic><sup>2</sup>=4.418, <italic>P</italic><0.05). The normal rate of blood glucose was 96.92% (63/65), which was higher than 86.36% (57/66) in control group (<italic>χ</italic><sup>2</sup>=4.474, <italic>P</italic><0.05). Conclusion:In addition to adipocytokines, modified Erzhu Erchentang could regulate adipokines of patients of PCOS with spleen deficiency and phlegm dampness, improve glucose, lipid metabolism and overweight, adjust endocrine hormone, reduce clinical symptoms and improve ovarian structure, so as to create conditions for conception.

OBJECTIVE@#To explore the objectivity and time-effect of stimulating effect at acupoint with PGLA in the healthy person, and to provide a basis for the rational interval of minimally invasive embedding of PGLA.@*METHODS@#Before embedding, 8 h, 3rd, 7th, 10th, 14th day after embedding, medical imaging magnetic resonance imaging (MRI) scanning technique was used to collect local T2WI pressure-lowering and T2-Mapping 8 echoes sequence image of left Zusanli (ST 36) in 8 cases of healthy person. The T2-Mapping 8 echoes sequence image was generated by the relevant software to the T2-Mapping image and the local T2 value was measured. The characteristics of local T2WI pressure-fat image signal intensity and the change of T2 value at left Zusanli (ST 36) with minimally invasive embedding with PGLA were observed and analyzed.@*RESULTS@#①There was no abnormal signal on the T2WI pressure-fat image on the left Zusanli (ST 36) point before the embedding. The high-signal was seen on the local T2WI pressure-fat image at each time point after embedding, there was no significant difference in local signal intensity between 8 h, 3rd and 7th day after embedding. The local signal intensity decreased on the 10th day after embedding, and the local signal intensity decreased significantly on the 14th day after embedding.②The T2 value at each time point after embedding increased significantly compared with that before embedding (all 0.05); there was no significant difference between the T2 value on the 7th and the 10th day after embedding (>0.05),the T2 value on the 14th day after embedding was significantly lower than that on the 7th day after embedding (<0.01).@*CONCLUSION@#It has a stimulating effect on the local acupoints with minimally invasive embedding with PGLA in the healthy person, and the stimulating effect has certain time-effect. The effective stimulation time is about 2 weeks. The rational interval period for the minimally invasive embedding with the PGLA of the same specification type should be about 2 weeks.

ABSTRACT

Objective To investigate the status and its influencing factors of job stressors in postpartum nurses with the second-child.Methods A total of 127 postpartum nurses with the second-child from 18 tertiary general hospitals in Beijing were investigated with general information questionnaire and the Nurse Stressor Scale.Results The scores of the Nurse Stressor Scale of postpartum nurses with the second-child were 487.99±209.37,mainly for the stressors of nature of nursing work,work load and family.Nature of the department,length of second-child maternity leave,satisfaction of income,appropriate adjustment and care of the department according to work position were the independent contributing factors(P<0.05).Conclusion The scores of the Nurse Stressor Scale of postpartum nurses with the second-child were in the middle level.Measures should be taken to ensure the length of the second-child maternity leave,arrange suitable work posts,raise salary,and give full care and support to postpartum nurses with the second-child in order to reduce their stress level.

ABSTRACT

Tetanic stimulation of the sciatic nerve (TSS) triggers long-term potentiation in the dorsal horn of the spinal cord and long-lasting pain hypersensitivity. CX3CL1-CX3CR1 signaling is an important pathway in neuronal-microglial activation. Nuclear factor κB (NF-κB) is a key signal transduction molecule that regulates neuroinflammation and neuropathic pain. Here, we set out to determine whether and how NF-κB and CX3CR1 are involved in the mechanism underlying the pathological changes induced by TSS. After unilateral TSS, significant bilateral mechanical allodynia was induced, as assessed by the von Frey test. The expression of phosphorylated NF-κB (pNF-κB) and CX3CR1 was significantly up-regulated in the bilateral dorsal horn. Immunofluorescence staining demonstrated that pNF-κB and NeuN co-existed, implying that the NF-κB pathway is predominantly activated in neurons following TSS. Administration of either the NF-κB inhibitor ammonium pyrrolidine dithiocarbamate or a CX3CR1-neutralizing antibody blocked the development and maintenance of neuropathic pain. In addition, blockade of NF-κB down-regulated the expression of CX3CL1-CX3CR1 signaling, and conversely the CX3CR1-neutralizing antibody also down-regulated pNF-κB. These findings suggest an involvement of NF-κB and the CX3CR1 signaling network in the development and maintenance of TSS-induced mechanical allodynia. Our work suggests the potential clinical application of NF-κB inhibitors or CX3CR1-neutralizing antibodies in treating pathological pain.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the synergistic antiproliferative and inducing-apoptotic effect of EPZ-5676 combined with chemotherapeutic drugs on acute lymploblastic leukemia(ALL).</p><p><b>METHODS</b>The MLL rearragement positive (MLL-r) ALL cell line RS4;11 was treated with EPZ-5676 alone and its combination with 5 kinds of chemotherapeutic drugs of ALL, the CCK-8 method was used to assay the inhibitory rate of leukemia cells treated with EPZ-5676 and chemotherapeutic drugs alone and their combination; the compusyn software was used to evaluate the relationthip between inhibitory rate (Fa) of combined drugs for leukemia cells and combination index (CI), and to determine the interaction of drugs; the flow cytometry with Annexin V-FITC/PI double staining was used to detect the apoptotic rate of RS4;11 treated EPZ-5676, GC, VCR, CTX, epirubicin and VP16 alone and combination of EPZ-5676 with them and to compare the inducing apoptotic effect of combined drugs.</p><p><b>RESULTS</b>The combination of EPZ-5672 with GC or VCR or VP16 showed synergistic antiproliferative effect; the combination of EPZ-5676 with TX or epirubicin displayed antogonistic effect. As compared with blank control group, 5, 10 and 25 µmol/L EPZ-5676 did not affected on apoptosis of RS4;11 cells, but 5 µmol/L EPZ-5676 combined with VCR of GC possessed synergistically inducing apoptotic effect (56.87% vs 12.93%)(P<0.01), (8.86% vs 5.28%)(P<0.05).</p><p><b>CONCLUSION</b>The EPZ-5676 combined with GC or VCR or VP16 possesses synergistic antiproliferative effect on RS4;11 cells, the effect of EPZ-5676 alone on apoptosis of RS4;11 cells is no significant, but the combination of low concentration EPZ-5676 with VCR or GC displays synergistically inducing apoptotic effect.</p>

ABSTRACT

Leukemia carring translocation at the 11q23 locus is referred to MLL-rearranged (MLL-r) leukemia, and the occurrence of this genetic lesion is associated with a poor prognosis. The most common translocation chromosomes are chromosomes 4,9 and 10. Recently MLL protein was found to interact with DOT1L (DOT1-like) protein, which can promote leukemogenesis. A new DOT1L inhibitor EPZ-5676 can selectively inhibit proliferation, promote apoptosis and differentiation, which was also found to act synergistically with anti-AML (acute myeloid leukemia) and anti-ALL (acute lymphoblastic leukemia) drugs. EPZ-5676 can also induce sustained regression in a rat xenograft model of MLL-rearranged leukemia. Now the novel drug is in phase I of clinical trail. The results suggest that the EPZ-5676 is promising. In this article, the mechanism of EPZ-5676 and its research progress are reviwed.

ABSTRACT

To study the coumarins of Anemone raddeana Regel, the compounds were separated by silica gel column chromatography and HPLC. Their structures were identified by their physicochemical property and spectral analysis. Two new compounds were isolated and identified as 4, 7-dimethoxyl-5-methyl-6-hydroxy coumarin (1) and 4, 7-dimethoxyl-5-formyl-6-hydroxycoumarin (2). The bioassays indicated that compounds 1 and 2 could significantly inhibit the proliferation of cancer cell, and showed the agonist effect on the transactivity of retinoic acid receptor-alpha (RARalpha). In addition, the two compounds had inhibitory effect against human leukocyte elastase (HLE).

ABSTRACT

Background: Ocular disorders have greatest potential for benefit from gene therapy. The major obstacle in the clinical application of gene therapy is not due to the lack of an ideal gene, but rather the lack of a clinically safe and efficient gene transfer method. Ultrasound (US) targeted microbubble destruction (UTMD)-mediated gene delivery system as a noninvasive gene transfer method is now widely used in gene therapy of cardiovascular disease, muscular tissue, and tumor, and proved to effectively enhance gene transfer in various studies in vitro and in vivo. However, it is just the beginning of application for ophthalmological disease. Objective: Review the latest advancements in UTMD-mediated ocular gene transfection and discuss mechanisms of UTMD involved in gene transfection, obstacles, and limitations to the use of this technology, as well as the perspectives for future applications of UTMD-mediated gene delivery system. Methods: Summarize published literature concerning UTMD-mediated ocular gene transfection. Results: UTMD is an effective and safe gene delivery method of therapy for ocular diseases. Considerable progress has been made in US or UTMD-mediated viral and nonviral ocular gene delivery to retina, like recombinant adeno-associated virus (rAAV) and nanoparticles as nonviral gene carriers. In addition, UTMD has potential for producing the blood-retinal barrier opening and serves as a promising method for intravenous ocular gene delivery. Conclusion: UTMD-mediated gene delivery system could effectively enhance gene transfer into ocular tissue. Though several problems remain to be solved, UTMD is a promising technology for the targeted gene therapy of ocular disease.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the impact of nutritional status on postoperative outcomes for patients with colorectal cancer.</p><p><b>METHODS</b>Data of 289 colorectal cancer patients from the Affiliated Hospital of Putian Medical College between January 2006 and December 2009 were collected prospectively. Nutritional status was evaluated according to Reilly Nutrition Risk Score(Reilly NRS) and Nutrition Risk Screening 2002(NRS-2002).</p><p><b>RESULTS</b>The postoperative mortality was 3.5%(10/289) and the complication rate was 29.4%(82/297). Patients were stratified into those at nutrition risk(n=89) and those not at risk(n=200) according to Reilly NRS and the two groups were similar in mortality rate(5.6% vs. 2.5%, P>0.05) and complication rate(36.1% vs. 26.5%, P>0.05). When stratified using NRS-2002, patients at nutritional risk(n=105) had a similar mortality rate (5.7% vs. 2.2%, P>0.05) but a higher complication rate(38.4% vs. 24.4%, P<0.05). NRS-2002 remained as an significant predictor of postoperative complications(P=0.007, OR=3.14, 95% CI:1.63-6.29) on multivariable logistic regression analysis.</p><p><b>CONCLUSION</b>As a nutritional evaluation tool, NRS-2002 may predict postoperative complication for colorectal cancer.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effect of different approaches of bone marrow stromal stem cell (BMSCs) transplantation into the allogenic rat liver.</p><p><b>METHODS</b>Thirty male SD rats were randomized equally into local liver group, portal vein group, and femoral vein group, and received injection of 1×10⁶/ml BMSCs directly into the rat liver, through the portal vein and through the femoral vein, respectively. The rat livers were scanned by magnetic resonance imaging (MRI) at 12 h and 1, 3, 5, 7, 14 days after the cell transplantation. Prussian blue staining of the rat liver sections was also performed 14 days after the transplantation.</p><p><b>RESULTS</b>MRI showed decreased signal intensity in all the rat livers of the local liver group; the ovoid area of the signal intensity gradually shrunk and the signal intensity increased with the passage of time. Lowered signal intensity was also seen in the rat livers of the portal vein group, appearing constantly branch-shaped, indistinct and increased gradually. Decreased signal intensity did not occur in the livers of femoral vein group. Prussian blue staining of all the rat livers at day 14 showed the presence of cells containing blue particles in all the groups, most numerous in the local liver group followed by the portal vein group and then the femoral vein group.</p><p><b>CONCLUSION</b>Direct intrahepatic injection of the BMSCs results in better effect than cell transplantation via the portal vein or the femoral vein.</p>

ABSTRACT

To study chemical constituents of Polygonatum odoratum (Mill.) Druce, the compounds were separated with column chromatography and HPLC. On the basis of physicochemical properties and spectral data, their structures were confirmed. Nine compounds were isolated and identified as 5,7-dihydroxy-6-methoxyl-8-methyl-3-(2',4'-dihydroxybenzyl)chroman-4-one (1), 5,7-dihydroxy-6-methyl-3-(2',4'-dihydroxybenzyl)chroman-4-one (2), 5,7-dihydroxy-6-methoxyl-8-methyl-3-(4'-methoxybenzyl)chroman-4-one (3), disporopsin (4), chrysoeriol (5), 5,4'-dihydroxy-7-methoxy-6-methylflavone (6), N-trans-feruloyltyramine (7), N-trans-feruloyloctopamine (8), and (+)-syringaresinol (9). Compounds 1-3 are new homoisoflavanones. Compounds 4-9 are isolated from this plant for the first time.

ABSTRACT

Objective To screen the antibacterial activity of Chinese traditional medicines against Yersinia pestis.Methods Six Chinese traditional medicines(Coptis Chinesis etc)were selected and extracted with pure water to make a concentration of 1 mg/L.Yersinia pestis strain 201 and EV 76 were used to determine the minimal inhibitory concentrations(MIC)of these selected medicines in vitro with liquid dilution method.Results Three herbs had inhibition effects on the strain 201 and EV76 in different extents,among which Rheum palmatum had the strongest effect and MIC was 0.025 00 mg/L.Furthermore,the Chinese traditional medicine had the same MIC on both strain 201 and EV76.Conclusions Chinese traditional medicines commonly used have inhibiting effect on Yersinia pesti.

ABSTRACT

<p><b>OBJECTIVE</b>To probe into the mechanisms of moxibustion and Chinese herbs in delaying aging.</p><p><b>METHODS</b>Sixty SD rats were randomly divided into a young control group, an aging group, a moxibustion group, a Chinese herb group and a moxibustion plus Chinese herb group. In the latter 4 groups, aging rat model was established by hypodermic injection of D-galactose. In the course of modeling, the 3 treatment group were treated by mild-warm moxibustion at "Zusanli (ST 36)", "Shenshu (BL 23)" and "Guanyuan (CV 4)" with reinforcing method, stomach perfusion of decoction of Liuwei Dihuang plus Danggui (Angelica) and Danshen (Salvia Miltiorrhiza Bge), and the moxibustion plus the Chinese herbs, respectively. After treatment for 40 days, liver mitochondrial DNA, serum IL-2 and IL-6 contents were detected.</p><p><b>RESULTS</b>(1) The mitochondrial DNA content of liver cells and serum IL-6 level significantly increased (P < 0.05, P < 0.01) and serum IL-2 level significantly decreased (P < 0.05) in the aging model group as compared with those in the young control group. (2) The mitochondrial DNA content of liver cells and serum IL-6 level significantly decreased (P < 0.05, P < 0.01) and serum IL-2 level significantly increased (P < 0.05) in all the treatment group as compared with those in the aging model group.</p><p><b>CONCLUSION</b>Moxibustion and Chinese herbs function delaying aging though decreasing mitochondrial DNA content of liver cells and serum IL-6 level and increasing serum IL-2 level.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the application of flow cytometry in diagnosis of T-cell rich diffuse large B-cell lymphoma.</p><p><b>METHODS</b>Histopathologic features, immunohistochemical findings and flow cytometry results of three cases of T-cell rich diffuse large B-cell lymphoma were reviewed retrospectively.</p><p><b>RESULTS</b>In CD45-side scatter (SSC) dot plot of the first patient, two different CD45-positive lymphoid cell populations were identified. The bright population consisted of both T and B cells, with a T-cell predominance. The dim population consisted mainly of B cells which showed lambda light chain restriction. In the second patient, CD45-positive cells were subdivided into two groups according to CD45-SSC dot plot. The small population consisted of both T and B cells, with a T-cell predominance. The large population consisted mainly of B cells which showed kappa light chain restriction. In the third patient, CD19-positive cells were subdivided into two groups according to the expression of CD20 in CD19-CD20 dot plot. The CD20-positive population expressed both kappa and lambda light chains, while the CD20-negative population demonstrated kappa light chain restriction.</p><p><b>CONCLUSIONS</b>Neoplastic B cells can be distinguished from reactive lymphoid cells in T-cell rich diffuse large B-cell lymphoma by flow cytometry, according to a number of parameters which include intensity of antigen expression, loss of antigens, expression of non-B-cell lineage antigens, patterns of forward scatter (FSC) and/or SSC, and expression of immature B-cell antigens.</p>

ABSTRACT

<p><b>OBJECTIVE</b>Nasal operation and/or H-uvulopalatopharyngoplasty (UPPP) was performed for obstructive sleep apnea hypopnea syndrome (OSAHS) patients with both oral pharynx and nasal obstruction, results analyzed.</p><p><b>METHODS</b>Patients were divided into group A (46 cases) and group B (42 case) randomly. Nasal procedures were: septoplasty, radiofrequency reduction of inferior turbinate, adenoidectomy and functional endoscopic operation. Cases in group A had nasal operation first, while cases in group B first had UPPP. All patients had sleep study with polysomnography (PSG) 2 and 12 months after each operation. Those who failed to reach the criteria of being effective after first surgery (defined as a 25% reduction in baseline apnea hypopnea index (AHI) received second phase operation (nasal operation for group B and UPPP for group A). The response rates were compared between the two groups after each phase of operation.</p><p><b>RESULTS</b>In group A, the phase one operation were effective in 44.0% (11/25) for the mild degree OSAHS patients (defined as AHI < 20/h), according to the sleep study performed 2 months after surgery, and no recurrence after one-year. All moderate ones (defined as 20/h < AHI < 40/h) responded poorly to nasal operation. The overall response rate was 23.9% (11/46). Non-responses (35 cases) in group A underwent UPPP and the response rate to it was 85.7% (30/35) in one year. In group B, UPPP operation was effective in 63.6% (14/22) mild cases and 30.0% (6/20) moderate cases in 2 months but 4 cases had recurrence in one year. Twenty-two cases underwent the second phase operation of nose and the response rate was 86.4% (19/22) in one year. There was no statistical significance on the overall response rate between group A and B (89.1% vs 83.3%, P > 0.05). While there was statistical significance of response rate between those patients who had only one operation and those who had both surgeries (P < 0.05).</p><p><b>CONCLUSIONS</b>The combination of nasal procedures and UPPP is effective a for OSAHS patient with nasal diseases especially in mild and moderate cases.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the flavonoids in the leaves of Apocynum ventum.</p><p><b>METHOD</b>Compounds were separated by repeated polyacrylamide column chromatgraphy combined with Sephadex LH-20 column chromatgraphy, their structures were confirmed by the analysis of spectral data and physicochemical properties.</p><p><b>RESULT</b>Six compounds were isolated from A. ventum. They were elucidated as quercetin (1), kaempferol (2), kaempferol-3-O-(6"-O-acetyl)-beta-D-glucopyranoside (3), quercetin-3-O-(6"-O-acetyl )-beta-D-glucopyranoside (4), quercetin-3-O-(6"-O-acetyl)-beta-D-galactopyranoside (5), kaempferol-7-O-alpha-L-rhamnopyranoside (6).</p><p><b>CONCLUSION</b>Compound 5 and 6 were isolated from the genus Apocynum for the first time.</p>
